What to have ready
- Your Medicare card (red, white, and blue), if you have one
- If we’ve worked together before — this year’s questionnaire, if it has arrived
- Your current plan card or paperwork, if you have one
- A list of your doctors and the hospitals you use
- Your prescriptions, including dosage
Don’t have all of these yet? That’s fine — we’ll work with what you have. Contact us first if you’d prefer.
Forms, if you need them
One form is required before we can talk about a Medicare plan— it’s a Medicare rule, and it applies every year. The others just make your appointment quicker. Don’t have a printer? No problem — we’ll have copies ready and go through them with you.

Medicare requires this before we can discuss any Medicare Advantage or Part D plan — every year, whether you're new or we've worked together for a decade. It only confirms which topics you've agreed to cover. It doesn't commit you to anything.
Medicare Advantage questionnaire
For clients on a Medicare Advantage plan (Part C). Your doctors, your prescriptions, and how you feel about your current plan.
Medicare Supplement / drug plan questionnaire
For clients with a Supplement (Medigap) plan, a stand-alone drug plan (Part D), or both.
