What to have ready

  • Your Medicare card (red, white, and blue), if you have one
  • Your current plan card or paperwork, if you have one
  • A list of your doctors and the hospitals you use
  • Your prescriptions, including dosage

Before your first appointment

One form is required before we can meet— it’s a Medicare rule. The other two are optional; filling them in ahead of time just makes your appointment faster. Don’t have a printer? No problem — we’ll have copies ready and go through them with you.

    Medicare requires this to be signed before we can discuss any Medicare Advantage or Part D plan. It simply confirms which topics you've agreed to cover — it doesn't commit you to anything. Fill it out and sign online in about two minutes.

    List your prescriptions and dosages here. It lets us check each plan's formulary, pharmacy network, and your real out-of-pocket costs before you decide.
