- Your current plan card or paperwork, if you have one
- A list of your doctors and the hospitals you use
- Your prescriptions, including dosage

Before your first appointment
One form is required before we can meet— it’s a Medicare rule. The other two are optional; filling them in ahead of time just makes your appointment faster. Don’t have a printer? No problem — we’ll have copies ready and go through them with you.

Medicare requires this to be signed before we can discuss any Medicare Advantage or Part D plan. It simply confirms which topics you've agreed to cover — it doesn't commit you to anything. Fill it out and sign online in about two minutes.
